About the Role:
Are you caring, compassionate and innovative and looking to make a difference to people’s lives? Join our Age UK Isle of Wight Community Services department working with the Integrated Discharge Team (IDT) based in St Mary’s Hospital, in a role that will help minimise delayed discharge from the hospital, focusing on how we can support patients to stay well, remain independent and be at home safely. The hours will be split between hospital and community working. In the hospital, you will speak to patients on the ward, to establish if any support may be needed when they go home. Following discharge from the hospital you will continue to support the patient, this could be with welfare visits, shopping and light domestic chores, prescription collections or transportation of patients’ belongings between their home/care home. This role includes weekend work on a rota basis.
About You:
Our patients are valued, and they all have a story to tell. You will have a genuine interest in and experience of helping older people using a person-centred approach, be able to work on your own initiative, with good organisational skills and to work both as part of a team and on your own. Collaboration with other organisations is essential to ensure each patient’s needs are met, so you will need to be able to engage with a diverse range of people. To deliver the community-based element, your own transport is essential and must be available every working day. Mileage will be paid for community visits. Due to the requirement for updating CRM systems, you will need to be secure in your IT skills. If you feel you meet these criteria, we want to hear from you.
